Terms of service

These Terms of Service (“Terms”) govern your access to and use of Co.Lab Health and the services we provide through it. By creating an account, booking a service, or otherwise using Co.Lab Health, you agree to be bound by these Terms. If you do not agree, please do not use the service.
1. Service description
Co.Lab Health is a platform for scheduling home healthcare — including diagnostics, wellness screening, vaccination, nursing, and online consultation services — delivered within our service area in Metro Manila. Field staff and clinicians are licensed medical professionals, and laboratory testing is performed by accredited Philippine laboratories. We may add, change, or discontinue specific services at any time.
2. Eligibility & accounts
You must be at least 18 years old to create an account and book directly. Services for minors or for another patient may be booked by a parent or legal guardian, who is responsible for the booking and for the accuracy of the information provided. You are responsible for keeping your login credentials confidential and for all activity under your account. Notify us promptly if you suspect unauthorized access.
3. Your responsibilities
When you use Co.Lab Health, you agree to:
- provide accurate, current, and complete information — including your identity, contact details, address, and any HMO, Senior Citizen, or PWD credentials you submit;
- be available at the booked address during your scheduled time window, or cancel/reschedule in advance as described below;
- follow any preparation instructions (for example, fasting) shown on your booking confirmation; and
- use the service only for lawful purposes and only for yourself or a patient you are authorized to act for.
4. Bookings, cancellations & no-shows
A booking request is confirmed only after our team verifies area availability and assigns staff. You may cancel or reschedule from your dashboard up to four (4) hours before your scheduled appointment at no charge. Cancellations made later than that, or appointments we cannot complete because no one is available at the address, may incur a small dispatch fee, since staff and single-use collection kits are already allocated to your visit.
5. Payments, pricing & refunds
Prices are shown per service or package at the time of booking and are payable in Philippine pesos. Direct payments are processed through certified Philippine payment partners; we do not store full card numbers. If you cancel a paid booking within the cancellation window, our team processes any eligible refund back to your original payment method. Refunds for completed visits are handled case by case in line with applicable law. A dispatch fee, where it applies, may be deducted from a refund.
6. HMO coverage
For HMO-covered services, you submit your HMO card and a valid ID, and we coordinate approval with your provider before the visit. If your provider approves the claim, no patient charge applies. If the claim is declined or incomplete, we will let you know your options, which may include proceeding as a direct payment. Approval is determined by your HMO provider, not by Co.Lab Health.
7. Senior Citizen & PWD discounts
Qualified Senior Citizens (Republic Act No. 9994) and Persons with Disability (Republic Act No. 10754) are entitled to a 20% discount on eligible services. To apply the discount, add your Senior Citizen or PWD ID at booking; we may require you to present or upload the ID for verification, and we may decline or reverse a discount applied with an invalid or unverifiable ID. The mandated discount applies to direct payments and does not stack with HMO coverage.
8. Results & medical disclaimer
Laboratory results are released to your patient portal and emailed to you once the laboratory issues them. Any information on this platform — including reference-range flags and AI-assisted chatbot responses — is for general guidance only and is not a substitute for professional medical advice, diagnosis, or treatment. Always consult a qualified physician about your results and before making any health decision. In an emergency, contact your local emergency services.
9. Acceptable use
You agree not to:
- submit false, fraudulent, or impersonating information, or book on behalf of someone without their authorization;
- attempt to gain unauthorized access to the platform, other users' data, or our systems, or to disrupt or probe them; or
- misuse, resell, or scrape the service, or use it in any way that violates Philippine law.
We may suspend or terminate accounts that violate these Terms or that we reasonably believe pose a risk to patients, staff, or the platform.
10. Intellectual property
The Co.Lab Health name, logo, software, and content are owned by us or our licensors and are protected by applicable law. You may use the platform only as permitted by these Terms. Your own data and results remain yours; you grant us only the limited rights needed to deliver the service.
11. Third-party services
We rely on third parties to deliver the service — including payment processors, accredited laboratories, HMO providers, and communication providers. Their handling of your information is described in our Privacy Policy. We are not responsible for the independent acts or omissions of these third parties beyond our role in coordinating your care.
12. Limitation of liability
To the maximum extent permitted by Philippine law, Co.Lab Health is not liable for indirect, incidental, special, or consequential damages arising from your use of the service. Nothing in these Terms limits any liability that cannot be limited under applicable law.
13. Indemnity
You agree to indemnify and hold Co.Lab Health harmless from claims and expenses arising out of your breach of these Terms, your misuse of the service, or information you submit that is false or that you were not authorized to provide.
14. Changes to these terms
We may update these Terms from time to time. When we make a material change we will update the “Last updated” date above and, where appropriate, ask you to accept the revised Terms before continuing to use the service. Your continued use after an update means you accept the change.
15. Governing law & disputes
These Terms are governed by the laws of the Republic of the Philippines. You agree to first contact us to resolve any dispute informally; if it cannot be resolved, disputes shall be subject to the exclusive jurisdiction of the proper courts of Metro Manila.
16. Severability
If any provision of these Terms is found unenforceable, the remaining provisions stay in full force, and the unenforceable provision will be applied to the maximum extent permitted by law.
